Appointments and schedule changes by phone
To consult a doctor at our hospital, you are required to have an appointment in advance.
For the first visit, the patient will have to be referred to us by their family doctor via Referral Documents and Application Forms for Appointment faxed by the referring medical facilities.
Please contact the Regional Medical Cooperation Center at 0956-24-1515 Ext.6148 if your family doctor is unable to fax in advance.*Available only in Japanese
For the return visit, please call the corresponding departments between 12:30pm-04:00pm weekdays. We accept appointments, cancelations and schedule changes up to 1 day or more in advance of your scheduled appointment date.
